What documents are needed for my appraisal?

For the appraisal, you will need your most updated CV and your supporting documents – CPD’s – which can be mostly covered by training courses such as Mandatory Training.

There is also an input form on the portfolio to complete.

In addition, we will need your patient and colleague feedback.
